STAR Registry Renewal Documentation
This document provides guidance on how to renew your STAR Registry entry. Please follow the instructions below based on your current STAR Registry status.
STAR Registry Renewal
If your answers on your Consensus Assessments Initiative Questionnaire (CAIQ) have not changed, you do not need to submit the same CAIQ to us for renewal. Instead, you may use the simplified renewal method.

Updated CAIQ Submission
If your CAIQ has been updated, you must submit your updated CAIQ as though it were a new submission. Please use this form to submit your updated CAIQ. Note CAIQ version 4 is the only version supported by this or the simplified process. Alternativley, you may upload your CAIQ using the new STAR Platform, Just login and access your CAIQ submission and then update your CAIQ in real-time at your convenience.
Auto Renewal
As long as your CAIQ is visible on the STAR Registry, you are eligible for auto renewal. This policy allows you to declare no changes in controls and auto renew your STAR Registry entry anytime prior to the three-year deadline.
Deprecation and Visibility Policy
Please note our policy regarding CAIQ deprecation and visibility:
- After one year, the CAIQ goes to Deprecated status.
- After three years, the CAIQ is dropped from public viewing.
It is crucial to renew your STAR Registry entry before it reaches the three-year deadline to ensure continuous visibility and compliance.
